Visa application for REPUBLIC OF DOMINICA

1. 2 photos 3.5 × 4.5 white background, no more than 6 months

2. Original passport + photocopied copy, valid for 6 months, signed, submitted with the old passport (if any)

3. Household registration book (photo of the whole book, true copy)

4. Birth certificate (copy y) + identity card.

5. Marriage / divorce certificate

6. Demonstrate financial:

Real estate documents, car (duplicate copy)

Bank account statement for the last 3 months (original with bank confirmation)

Confirmation of current balance of ATM card or savings book (original with bank confirmation)

7. Proof of work:

** If Business

- Business license (true copy)

- Receipt of tax payment of this year and the previous year (true copy)

- Statement of operational account of the company for the latest 3 months (the original has a confirmation from the bank)

** If you are an Employee

- Labor contract (true copy or certified copy of the company)

- Payroll of the last 3 months (copy, with a company seal) or confirmation of the latest 03 months salary (original certified by the company)

- An application for leave (original, certified by the company) clearly stated during the leave

** If the student

- Certificate of student's school

- Letter of absence (if during the school schedule)

- Written consent for the child to travel by parents with local confirmation (according to the form of consular)

- Certified copy of parent's passport (copy true copy)

- Payment confirmation

- Letter of admission to the school in the Dominican Republic

*** Without parents accompanying additional work and financial parent

** If retired:

- Decision to retire
